Expert consensus on perioperative urosepsis nursing of patients with intracavitary surgery for urinary tract calculi

Objective To establish a consensus on perioperative urospesis nursing of patients with intracavitary surgery for urinary tract calculi. Methods A consensus developing group was established,and the first edition of the "Expert consensus on perioperative urosepsis nursing of patients with intracavitary surgery for urinary tract calculi"(hereinafter referred to as the "consensus") was formed after a large amount of literature review and collation,online expert consultation and group discussion. Domestic medical experts from urology,minimally invasive operating rooms and intensive care units were invited to discuss and revise the "consensus" for 2 rounds,and then peer review was conducted,and finally the "consensus" was completed. Results The final version of the "consensus" comprises 3 major sections,namely the preoperative,intraoperative and postoperative,which covers high-risk factors,prevention,identification,condition monitoring and nursing intervention,with a total of 21 items. The positive coefficients for the 2 rounds of consultations were 100%. The coefficients of judgment,familiarity and authority in the 2 rounds of consultations were 0.975,0.960 and 0.968,respectively. In the 2 rounds of expert consultations,the importance assignments of each items were(4.847±0.304) and(4.888±0.212),and the overall Kendall’s coefficients were 0.232 and 0.287,respectively. Conclusion The "consensus" integrates the suggestions of multidisciplinary experts with a certain degree of practicality,representativeness and scientificity. It can provide guidance for clinical nursing practice of patients with perioperative urosepsis of intracavitary surgery for urinary tract calculi.

Construction of nursing intervention system for patients with type 2 diabetes and osteoporosis

Objective To construct a feasible,systematic and standardized nursing intervention system for patients with type 2 diabetes and osteoporosis,and to provide relevant evidence. Methods The preliminary intervention system was drafted by searching literature on the basis of relevant guidelines and case reviews,and the Delphi method was adopted to finalize the system in accordance with expert advice and clinical experience. Results Totally 15 experts were enrolled,and the opinions of all experts tended to be consistent after 2 rounds of consultation. The expert authority coefficient was 0.862,and the importance and feasibility of Kendall's coordination coefficients were 0.369,0.396 and 0.428,0.415 in the 2 rounds respectively(P<0.001). The content included 6 sections for first-level indicator,namely nutrition intervention,exercise intervention,disease-related knowledge education,medication guidance,pain nursing intervention,and psychosocial intervention;it also contained 19 second-level indicators,and 59 third-level indicators. Conclusion The content of nursing intervention system is scientific,reliable,practical and pertinent,and is of great significance for clinical nursing practice.

The intervention effects of fracture liaison service in elderly patients with hip fragility fractures

Objective To explore the intervention effects of fracture liaison service in elderly patients with hip fragility fractures.Methods Totally 106 patients with hip fragility fractures were selected in a tertiary hospital from March to July 2018. The patients were numbered according to the admission order. The odd-numbered digital grasping method was used to determine that patients with the odd numbers were included in the experimental group(n=53),and those with even numbers were included in the control group(n=53). The experimental group was given fracture liaison service on the basis of routine nursing and the control group was given routine nursing. The degree of frailty(Elderly Frailty Assessment Scale),osteoporosis diagnosis rate and secondary fracture incidence were compared between two groups.Results After 3 months of intervention,the scores of physiological and psychological frailty dimensions in the experimental group were lower than those in the control group. After 6 months of intervention,the scores of physiological,psychological,cognitive dimensions and frailty total score in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05). Repeated measurement analysis of variance results showed that the inter-group effect,time effect and interaction between the two groups of physiological,cognitive dimension score and frailty total score were statistically significant(P<0.05). After 3 months and 6 months of intervention,the diagnosis rate of osteoporosis in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.05). There was no significant difference in the incidence of secondary fracture between two groups during different intervention periods(P>0.05).Conclusion Fracture liaison service can reduce the degree of frailty in elderly patients with hip fragility fractures,and improve the patients’ osteoporosis diagnosis rate.
